Travelers can obtain their SMART Health Cards from any organization that has their clinical information, such as a pharmacy, doctor’s office, or state registry.
If they are vaccinated at a pharmacy or medical institution that issues SHCs, they can request from them directly (ie. logging into pharmacy portals of Walmart, Sam’s Club or CVS Health)
If they are vaccinated in a state that issues SHCs, they can get it from their state. They can check with their state or county’s immunization registry. States such as New York (Excelsior PLUS pass only), California, Hawaii and Louisiana have made SMART Health Cards available to residents via online portals. Some counties in Maryland have also done so.
If both platforms are not applicable,

- Medical institutions within the EPIC system might be connected to the vaccine registry of the state that they are operating in.
- If they have an existing relationship with one of those providers, they can try asking their doctor to check the state’s vaccine registry and issue a SHC for them.
